With this contact form you can restrict phone field to numbers only and use tab control.
It works with a simple php script that send all variables to your mail after validation of required fields.
Easy to change and you just need drag and drop in your project because all content is in one mc and _root is not used. Extra two nice sounds (roll over and click).
